Maintain contact with referral sources, Assist clients with other social service needs-housing/shelter, mental health care, and medical care, If client is ineligible for county funding, work with client on alternatives for treatment. Non-WIser Choice screens should be completed and all available resources should be given to the client i.e. sign the client up for food share, medical care, Monthly case review and closing of vouchers, Adheres to IMPACT values of team, customer service and value added, Ensures that IMPACT is compliant with applicable federal, state, and local laws ensuring client confidentiality.

Case Manager Interview project participants to identify problems, needs and situations: violence and abuse prevention, conflict management, familiy and couple, prevention of use and abuse of drugs and alcohol, strategies of abstinence, contraception, prevention of the spread of the Sexually Transmitted Infections, among others.
