HEALTHSPRING OF ALABAMA, Birmingham, AL Program Coordinator Developed the Life Enhancement Program to identify and address the unique needs of Medicare members with terminal illnesses to deliver cost effective, quality-based care in the appropriate setting. Established program policy and procedures, criteria for admission to the program, metrics for evaluation and physician relationships to obtain timely referrals. Recruited, interviewed, hired trained and retained staff according to the staffing model. Responsible for $150,000 cost savings within the first month of establishing the Life Enhancement Program through recognition of high utilization and 30 day readmissions of patients that were hospice appropriate. Successfully trained and garnered the support of 8 of 10 physician groups and increased hospice referrals from less than 10% to over 60% within 3 months of program implementation. Decreasedregional readmission rates from 32% to 18% within the first 6 months of the program. Provided direct supervision of 10 FTE's both licensed and non-licensed.
