Easter Seals Tristate
Meets with consumers to complete intake interviews, monitor progress for 30 day assessments, coordinate staffing with family/service facilitators, and review progress while also administering vocational interest testing. Develops, implements, and monitors annual individualized and unified plans. Provides continued case management to all consumers by completing/filing all documentation. Reviews all documentation sheets on clients for accuracy, completeness, and necessity, while completing any training one may request. Serving up to 75 individuals at a time.
