Provide and implement direct and indirect Nursing Services to Individuals, Family, community groups and home care settings.
Identify physical, social and emotional needs of the community and implement comprehensive care plans.
Educate and counsel patients with Chronic illness
Initiate and conduct home care visits
Research, Develop and Maintain community resources and program's for clients and family.
Conduct immunization to adults and children
Developed strong community relations through networking and liaison representation -Diabetic Association, Hospice care, Cancer Society.
In charge of Medical team at Emergency/Hurricane Shelter.
Prepare and conduct in service education presentations in health related areas to medical personnel and the community.
Patient advocate, assessed patient status and notify Physicians of clinical changes
Implement total patient care as first responder to medical emergencies
Coordinate services, treatment, and care as directed and specific to the patients needs
Educate patients and caregivers on health, wellness, parenting and home management, medication
administration, wound care and other areas as needed.
Provide skilled nursing care in medication administration, Phlebotomy, IV infusion, wound care.
Overall responsibility to provide palliative care in outpatient settings ranging from assessment to development and implementation of care plans, for the provision of health care to a community of
approximately 3000 residents which includes 2 Schools and a Geriatric Residential Home.
Member of Quality Assurance, Health and Safety , Disaster Preparation Committee and Ethics Committee
Super user and trainer - Cerner Medical Reporting System.
