Coordinating Council for Independant Living

2010 - Present- Work as Personal Care Homemaker RN.  Responsible for scheduling home visits, completing and processing all paperwork to maintain client Personal Care services.  Complete nursing assessments, home visits, referrals to other agencies and oversee direct care services.

2009 -2010 -Coordinate with West Virginia Choice(WVC) Manager and Admission RN to determine areas where Direct Care Workers (DCW) are needed and schedule training dates. Work with Administrative WVC trainer to provide off site training new potential DCW's. Training include DVD instruction, verbal instruction with discussion, written tests covering all areas of patient personal care. This training also includes CPR and First Aid training. Track and inform current DCW's of ongoing training needs. Schedule and send written documentation of upcoming off-site training availability to ensure up-to-date compliance.

HealthSouth Western Hills Rehabilitation Hospital

Developed a specific marketing area, making contacts with local physicians, senior community groups, senior care facilities, case managers and discharge planners.  Provided educational information on health subjects, rehabilitation services available and generated new referrals from the focus sources.  Completed assessments and evaluations for new potential clients for admission to hospital. 

Developed HealthSouth Western Hills Newsletter; created and taught power point presentations on Fall Risk/Safety, Medication Management and Senior Concerns to senior community groups.  Developed and maintained Spreadsheet/Excel of Non Conversion clients for contacts.  Developed and maintained Excel database of patients discharged to make follow-up calls at 30-60 day increments.

Eagle Pointe Skilled Nursing Facility

Managed care and treatment of residents in a 63-bed unit.  Supervised LPN's, CNA's, and ancillary staff in performance of specific duties in resident care.  Maintained documentation, reviewed charts, made contact with physicians and care providers when needed and created  an environment for the staff within the unit. 

Provided personal bedside care to residents, which includes but was not limited to:

Made on-site assessents of any critically ill residents, consulted with physician and bedside nursing care and provided treatment as needed. 

Scheduled and coordinated lab work for residents according to physician ordered dates and called any critical lab reports to physician and faxed all reports with pertinent informatin regarding resident medical history to physician.

Worked with other disciplines /staff members to create a cooperative and positive working environment for staff.

Provided case management services for 30-100 clients in behavioral health setting.  Scheduled appointments for treatment team review with clients and family members at 3-month intervals or sooner if there were any life changing events.  Scheduled and insured compliance with appointments to Psychatric physician, psychologist, counselors, and case managers. 

Treatment Team consisted of reviewing current treatment plan for client, services received, compliance with treatment, positive outcomes, negative outcomes, any changes in life/family environment and what changes needed to be made for next 90-day treatment plan.
