Complete psychosocial assessments, crisis and short-term counseling, community resource education, process and educational groups, discharge planning for patients/families in inpatient and outpatient settings. Provide resource on psychosocial issues and community services to physicians and staff. Files petitions with the court as directed by the physician. Assist patients/families with their psychosocial needs. Provides education to patients/families about available options for care, utilizing community resources.  Assists patients with financial concerns and basic needs which impact their mental health. Facilitates post-hospitalization placement (i.e. nursing home, group home, assisted living, etc.). Intervene when neccessary in psychosocially complex case requiring a high degree of clinical judgment (i.e. child/elder abuse, crisis intervention, grief and loss issues, psychiatric conditions, family relationship problems, etc.) providing counseling and /or appropriate referrals.  Collaborates with interdisciplinary team members and represents patients' psychosocial needs to the rest of the healthcare team.  Document assessments, plans, and interventions in medical charts. Lastly, complete the process of discharging patients from the hospital back into the community.

Interests

I have a passion to help others make a difference in their lives. I would like to be that voice for those that are voiceless, provide counseling to the mental ill population; work with children that come from a broken home and/or struggling in school.  I would like to be that advocate who make change for the homeless people that resides in our society.  My interest is to be the best social worker that I can be and continue to educate myself on the different resources that are available in our community.
