Social Service Director-Follow the MDS calendar for assessments and notes for charts In charge of the Behavior Program of the residents Plan and schedule the following Dr. visits for the facility: (Eye Dr. ,POD,Care Now Services), Any Dr/Dental appointments and transportation outside of facility, Shopping for the residents, Referrals for home health upon discharge, Set up discharges, Grievances, Missing Items, Assist residents with transition of nursing home placement, I am available to all residents as needed
