LPN/ Weekend On Call

As the weekend nurse, my responsibilities are to see all patients scheduled for the weekend, manage all phone calls, referrals and scheduling for additional patients for start of new week. I monitor the patients with home telemonitors, perform any necessary procedures; Foly catheter replacement, IV administration, patient teaching, sq and IM injections, patient assessment for problems, notifying MD of any pertinant problems that arise over the weekend.

  This job requires a great deal of independance and time management skills. I have been able to work for the last year without a full time RN backing me up. The weekday nurses have been having to fill in while we searched for a full time replacement for our weekend RN. Our previous nurse became ill and has been unable to return to work. This has created more responsibility for me in caring for the patients and notifying the proper individuals for assistance.

  During our transition to EMR, I have taken on more responsibility in working during the week to assist the supervisory staff in transcribing patient care notes, admissions and PT/OT notes from paper to paperless documentation.

LPN/Gateway to care coordinator

As the Gateway to care coordinator, my responsibility was the management of all patients, within our clinic, on the Gateway to care program. Gateway was a medication assistance program that the hospital, in conjunction with leading pharmaceutical companies,assisted patients with obtaining their medication when they were unable to afford medication. During the time that I was coordinating the program for the family practice office, I managed medications and applications for 450 patients.

  My responsibilities within the organization were in maintainence of the sample drug room, coordinating pharmaceutical rep. appointments and assisting physicians in clinic as a staff nurse. During my employment at Phobe Family Care, I wore many hats. I worked in phone triage and assisted in scheduling of patient appointments, I was the medication refill nurse, prior approval authorizations and triage of walk ins. I assisted in resident training, nurse education and complied a nursing resource manual for the nursing staff.

During my employment there, I won the Nurse of the Year award, an award for volunteer work with Hurricane Katrina victims and was active in the American Red Cross as a volunteer.

 As staff nurse in a busy urgent care facility, my responsibilities were in triage and assessment of all patients, assisting physicians with examinations and procedures. My responsibilities were also to manage staff schedules,ordering of supplies and pharmacy items, assisting in obtaining drug/BAT tests for DOT and non-DOT occupational medicine patients. We performed all after hour lab tests, drug screenings/BAT for  post accidents.

 I served as the interim Assistant Nurse manager for occupational health from April - December 2003 . I managed schedules, assisted physician, assisted with workers compensation exams, occupational hearing testing, pulmonary testing, vision exams and Secret Service Physicals, and drug screen reporting. During this time, I became a certified occupational hearing technician(CAHOC). I received numerous commendations from my peers for outstanding leadership and nursing care.

As staff nurse, I assisted physicians in care and treatment of female patients and all of their health care needs.

I assisted in lab procedures; drawing blood, performing CLIA waived lab testing, obtained history on patients, assisted physician in exams and procedures performed in office: Leep Procedure, colposcopy, and ultrasound.

I was responsible for recording and reporting lab results, notifying patients of normal lab results, confirming appointments, returning patient phone calls and telephone triage of OB patients.

During my employment with Phoebe Putney, I was employed in the ICU step down unit, SICU, and Emergency departments. Under the direction of the physicians and RN's, I performed patient care to those patients assigned to me. I enjoyed working with the intensive care patients and those we cared for in the emergency room. I maintained my ACLS certification while working in the critical care areas. I recieved extra training and instruction in critical care throught the LPN II program. In 1990, I received the Caring Hands Award for exceptional care and service.

CLIA Waived Testing / LAB Skills
During my employment at Phoebe Convienent Care Center, the staff was responsible for start up/shut down of all lab equipment as well as performing CLIA waived testing; CBC, Chemistry studies, Rapid Strep, mono testing, pregnancy testing and all other veinipunctures that required send out to hospital lab.
