Present Employment and Experience Details:

I had worked as “Supervisor” and “Field Investigator” in the different project under the guidance & supervision of department of Community Medicine, Upgraded Department of Social & Preventive Medicine (SPM), KGMU, Lucknow and presently working with Institute of Applied Statistics and Development Studies (IASDS), Lucknow.

PROJECT EXPERIENCE

·“Maternal and Child Health Nutrition Program” – A community based survey of UNICEF organized by Upgraded Department of Social and Preventive Medicine (SPM), King George’s Medical University, Lucknow. In this project 20 villages of Lucknow district were surveyed to assess chiefly nutritional status of Mothers and Children in rural community along with immunization status, awareness about family planning, food habits and weaning pattern of baby.

·Survey work for evaluation of health benefits of “SWAJAL PROJECT” – A project of WORLD BANK. In this project 14 villages of hill areas of Almorah district and 10 villages of Jhansi District were surveyed to assess it’s effect especially magnitude of diarrhoeal diseases and status of personal hygine in children up to five years of age. Baseline survey and first repeat survey in Almorah district was carried out by me in September 2000 and 2001 respectively and second repeat survey was carried out by me in 2002 in Jhansi district. Survey organized by Upgraded Department of Social and Preventive Medicine (SPM), King George’s Medical University, Lucknow.

·“Assessment of Disability in Project Area” – An ICMR project. In first phase of this project one lakh population residing in about one hundred Aanganwadi cetres of Community Block Sarojini Nagar, District Lucknow were screened to find out the magnitude of disability prevailing in children upto six years of age. Duration from March 17, 2001 till August 31, 2001. Assessment organized by Upgraded Department of Social and Preventive Medicine (SPM), King George’s Medical University, Lucknow.

·Study on issues involved in various high-risk groups in perspective of HIV/AIDS control strategies in district of Uttar Pradesh. An AIDS Control Society project. In this project both rural and urban areas of Unnao district were covered by me to assess the knowledge and awareness about HIV/AIDS/STD and its preventive measures in rural and urban community. Organized by Upgraded Department of Social and Preventive Medicine (SPM), King George’s Medical University, Lucknow.

·Worked in MOST - US AID Project “Situational Analysis of Iron Deficiency Anemia and Vit. ‘A‘” jointly organized by Institute of Applied Statistics and Development Studies, Lucknow and Upgraded Department of Social and Preventive Medicine (SPM), King George’s Medical University, Lucknow. In this project 8 districts of Uttar Pradesh have been taken for study. The target groups being Pregnant Women and Lactating Mothers and Children 0-5 years of age of the selected blocks.

·Validation of reported coverage of Immunization, Antenatal Care (ANC) and Postnatal Care (PNC) in Hamirpur District during February-March 2003. A UNICEF sponsored project. Organized by Upgraded Department of Social and Preventive Medicine (SPM), King George’s Medical University, Lucknow.

·Baseline survey of Iodine Deficiency Disorder under National Iodine deficiency Disorder Control Program, funded by GOI and UNICEF. Place: Bahraich (10 urban and 10 rural PSUs) from April 2003 till May 2003. Survey organized by Upgraded Department of Social and Preventive Medicine (SPM), King George’s Medical University, Lucknow.

·Reproductive and Child Health (RCH) Survey 2004 organized by KGMC-ICE, King George’s Medical University, Lucknow in Raibareily and Firozabad District (June – July 2004).

·Study on Drug Abuse in Prison’s of Uttar Pradesh. Organized by Director General Prisons and Correctional Services, Uttar Pradesh. Place: Central Jail Gorakhpur in August 2004.

·CARE-INDIA - Neonatal Evaluation Research Project – Adequacy Survey Round II - 2004, Place – Barabanki and Unnao, During August – September 2004. Jointly organized by King George’s Medical University and John Hopkins University.

·CARE-INDIA (Madhya Pradesh) - Semi Annual Assessment of Maternal and Infant Survival Project (MISP). Place: Block Timarni, District Harda, State Madhya Pradesh in September 2004. Organized by Institute of Applied Statistics and Development Studies, Lucknow.

·Validation of Educational Data. A UNICEF sponsored project. Organized by Department of Education, University of Lucknow. Place: Block- Ram Nagar, District- Basti, State- Uttar Pradesh. Duration- November 01, 2004 till November 06, 2004.

·Baseline survey on Impact assessment of ICDS Food Fortification. A World Food Program (WFP) Project. Organized by Institute For Research in Medical Statistics (IRMS) – ICMR, New Delhi. Place: Block- Paonta Sahib, District- Sirmaur, State- Himachal Pradesh. Duration- December 02, 2004 till December 19, 2004.

·CARE INDIA (Madhya Pradesh) - Final Assessment of Maternal and Infant Survival Project (MISP). Blocks: Pipariya, Seoni Malwa, Sohagpur, Timarni, Narsinggarh, Obedullahganj and Phanda. Districts: Hoshangabad, Harda, Rajgarh, Raisen and Bhopal. Duration: from January 07, 2005 till February 01, 2005. Organized by Institute of Applied Statistics and Development Studies, Lucknow.

·Baseline survey on Impact assessment of Nutritional Intervention through Food for Education (FFE) in India. A World Food Program (WFP) project. Place: Blocks- Kalsi, Chakrata, Bhilangana and Jaunpur, Districts- Dehradun and Tehri Garhwal, State: Uttaranchal. Duration: from February 18, 2005 to March 24, 2005. Organized by Institute of Applied Statistics and Development Studies, Lucknow.

·Baseline survey on Impact assessment of Nutritional Intervention through Food for Education (FFE) in India. A World Food Program (WFP) project. Place: Blocks- Pathalgaon, Kunkuri, Kharsia and Gharghoda, Districts- Jaspur Nagar and Raigarh, State: Chhattisgarh. Duration: from April 04, 2005 to April 30, 2005. ·Goiter Surey in Balrampur District. Place: 9 rural blocks + Urban, from May 09, 2005 till May 15, 2005. Conducted by Upgraded Department of Social and Preventive Medicine (SPM), King George’s Medical University, Lucknow.

·Study on Prevalence of Goiter among Male and Female of all age groups in Faizabad, Shrawasti, and Moradabad districts of Uttar Pradesh in February, March and May 2007.
